My Consent Forms is a digital consent and client record management platform for aesthetics and wellness practitioners. It provides customisable treatment consent forms, digital signatures, client management, before-and-after photo tracking, and treatment-specific aftercare.

Practitioners using this platform agree to:

• Provide accurate clinic and contact details
• Obtain valid, informed consent from clients before treatment
• Ensure all client information entered is accurate and necessary
• Maintain the lawful basis for processing client personal and health data
• Keep login credentials secure and not share them with unauthorised users
• Use the platform only for legitimate clinical and business purposes

Clients completing consent forms agree to:

• Provide accurate and truthful information about their medical history and suitability
• Read the treatment information, risks, and aftercare instructions carefully
• Acknowledge that they understand and consent to the described treatment
• Inform their practitioner of any changes to their medical history before treatment

Completing a digital consent form through this platform documents a client's informed consent. It does not replace a practitioner's professional duty of care or clinical judgement. The practitioner remains responsible for assessing suitability, explaining the treatment, and delivering safe care.

Digital signatures include a timestamp and IP address for audit and verification purposes. A completed consent form is a record of the client's consent at the time of signing and should be retained according to the clinic's retention policy.

Personal and health data collected through this platform is handled in accordance with our Privacy Policy. Practitioners are the data controller for client data. My Consent Forms acts as a data processor and does not use client data for its own purposes.

The treatment consent form templates, aftercare instructions, and related content in the My Consent Forms library are proprietary to My Consent Forms. Practitioners may use these templates within the platform for their own clinical practice. Bulk scraping, downloading, redistributing, or reselling the treatment library content is strictly prohibited.

Practitioner access to premium features is provided through a subscription plan ("My Consent Forms Premium"). Subscriptions renew automatically unless cancelled at least 24 hours before the end of the current billing period. To cancel, contact Base44 support through the app. A 7-day free trial may be offered to new subscribers.

My Consent Forms provides the platform as a tool for documenting consent and managing client records. We are not a healthcare provider and do not give medical advice. We are not liable for clinical outcomes, treatment decisions, or the accuracy of information entered by practitioners or clients. Our liability is limited to the maximum extent permitted by law.

We may update these Terms & Conditions from time to time. Continued use of the platform after changes are posted constitutes acceptance of the updated terms.
